You can pick up a free CRITICAL Conditions^SM Planning Guide at any of the following locations:

Ø	The Marketing Department in ANNEX at Satilla Regional Medical Center located at 410 Darling Avenue, Waycross.
Ø	Ware County Senior Center, located at 1615 Carswell Avenue, Waycross.
Ø	Ware County Health Department, 604 Riverside Avenue, Waycross.
Ø	Georgia Legal Services, 1057 Grove Avenue, Waycross.

You can register for a CRITICAL Conditions^SM Workshop or schedule an appointment to discuss final health care planning with a Community Counselor by calling 912/287-2573.
