You can pick up a free CRITICAL Conditions^SM Planning Guide at any of the following locations:

Ø	The John D. Archbold Memorial Hospital, Gordon Avenue at Mimosa Drive, main lobby information desk, Thomasville.
Ø	In the Patient Representative Office or information desk of John D. Archbold Memorial Hospital, Thomasville.
Ø	Archbold Home Health, 400 Old Albany Road, information desk, Thomasville.
Ø	The Thomas County Health Department located at the Hancock Human Service Center, 440 Smith Street, Thomasville.
Ø	The Thomas County Extension Office, FACS Agent, located at the Elijah Hill Governmental Building, 227 West Jefferson Street, Thomasville.
