You can pick up a free CRITICAL Conditions^SM Planning Guide at any of the following locations:

Ø	Mitchell County Hospital, 90 E. Stephens Street, Patient Representative's Office, in Camilla.
Ø	Mitchell County Health Department, 88 W. Oakland Avenue, Camilla.
Ø	Pelham Senior Center, 308 W. Railroad Street in Pelham.

You can register for a CRITICAL Conditions^SM Workshop or schedule an appointment to discuss final health care planning with a Community Counselor, by calling Mitchell County Hospital, Patient Representative's Office, at 229/336-4621.
