Ø	Lakeland Villa Convalescent Center, Social Services Department, 888 W. Thigpen Ave., Lakeland
Ø	Lanier County Health Department, 205 W. Murrell St., Lakeland

You can register for a CRITICAL Conditions^SM Workshop or schedule an appointment to discuss final health care planning with a Community Counselor by calling 229-482-8537
