You can pick up a free CRITICAL Conditions^SM Planning Guide at any of the following locations:

Ø	Grady County Senior Center, 33 17th Avenue, NW, Cairo.
Ø	Grady County Health Department, 1030 4th Street, SE, Cairo.

You can register for a CRITICAL Conditions^SM Workshop or schedule an appointment to discuss final health care planning with a Community Counselor, by calling Grady General Hospital, business Office at 229/377-1150.
