Community Counselors are individuals who have been trained to provide guidance on final health care planning. They will meet one-on-one, free of charge, with individuals or families and can: assist in overcoming barriers to discussing end-of-life issues with loved ones and heath care providers facilitate family discussions on final health care planning using the CRITICAL ConditionsSM Planning Guide help individuals and/or families think through their final health care decisions provide clarification on terminology associated with final health care planning answer questions and assist individuals in completing a Georgia Advance Directive for Health Care.
