| CRITICAL ConditionsSM is a community educational program designed by Georgia Health Decisions to help people better understand and plan for their health care at the end of life. The program features: · Public Awareness Materials - an array of materials applicable in a variety of settings to educate the public about the importance of planning for their final health care. · The CRITICAL ConditionsSM Planning Guide a discussion tool that uses real-life scenarios to help people talk about their final health care choices with family members and record their wishes using the Georgia Advance Directive for Health Care. · CRITICAL ConditionsSM Family Workshops that explain the process of making final health care decisions using the CRITICAL ConditionsSM Planning Guide. · Trained Community Counselors who meet one-on-one with individuals and families to talk about final health care issues, answer their questions, and help them make better-informed decisions. · Training and Orientation Modules that familiarize staff of hospitals, physicians and other health care providers with the CRITICAL ConditionsSM process and public perception of end-of-life issues. From July, 1998 through June, 2000 the CRITICAL ConditionsSM program was implemented in eight pilot communities across Georgia in order to develop effective models for helping individuals and families discuss and document their final health care wishes. The program is currently being made available across Georgia through the enrollment of health systems or other organizations wishing to sponsor the program in their communities. |
